WIAA Physical Form / PRINT SPORTS PHYSICAL FORM HERE 
  • Physical Cards
    • All students must have an active physical on file in order to participate and is good for two years. 
      • This card must be signed by a physician to verify that the student has had a physical within the past two years and is being allowed to participate in athletics.  If the physical was done after April 1st, it will be good for the next two full school years. Parent/Guardian signature is also required.
Students may not participate until this card is turned in.
  • Dates of physicals are available on Skyward in “Health Info” (click display options: vaccinations or physical exams)
