Registration, Eligibility, Paperwork


Registration / Sports Meetings Scheduled include: 

October 21, 2018 Winter Sports Meeting (2018-19) 6pm
February 17, 2019 Spring Sports Meeting (2018-19) 6pm
May 19, 2019 Fall Sports Meeting (2019-20) 6pm
 (Note: Athletes competing in either Gymnastics or Boys' Swim should attend their meeting at Memorial. Same days/times apply.)
                                       
Large group meeting in the auditorium start at 6pm and team meetings begin at approximately 6:30.

The WIAA determines the earliest start date for each sport. Refer to this WIAA calendar for more information

 
Athletic fees can be paid at these meetings as a member of the district finance office will be on site.  After this meeting, fees will continue to be paid at the District Office during summer business hours, prior to the start of fall sport practice. Call the North Athletic Office at 715-852-6678 with questions.

Student Accident Insurance: For any NHS athlete interested in obtaining voluntary student accident insurance, please refer to this insurance agency for information.

The two documents below will explain eligibility requirements as pertain to both the WIAA and the Eau Claire Area School District Athletic Code.

Paperwork
After reviewing the above eligibility documents, student athletes and parents/guardians will need to complete the seven paperwork requirements listed below.  No student athlete will be allowed to try out, begin practice, or participate in any sport until all paperwork is completed.  All forms listed below are also available to be picked up at North High School. 

All forms should be returned to the North High School Athletic Department with the exception of the Athletic Fee Payment form which is turned into the Accounting Department, 500 Main Street.

Athletics Permission Slip
Requires the signature of both student athlete and parent/guardian. Be sure to complete the entire form. Incomplete forms will be returned for completion prior to participation.

Athletics Emergency Waiver
Requires the signature of the parent/guardian on one side and the signature of both the student athlete and the parent/guardian on the reverse side

Athletic Code Card
Requires signature of both student athlete and parent/guardian and indicates an understanding of the ECASD Athletic Code and WIAA Eligibility bulletin.
     - The WIAA Eligibility bulletin should be read by all student athletes prior to participating. You do not need to sign the document as your signature on the ATHLETIC CODE CARD (PER BOARD POLICY 6145) INDICATES AGREEMENT TO ABIDE BY ALL OF THE INFORMATION CONTAINED IN THIS BULLETIN.

Athletic Fee Payment Form
Requires signature of parent/guardian. The form itself contains all sports' fee amounts. Forms/payments need to be returned to the district's central office; the address is included on the form.

WIAA Physical Form
To be eligible for participation in sports for the 2018-19 school year, physicals on file in the North High athletic office must be dated after April 1, 2017.

NHS IMPACT Program - Head Injury Questionnaire
This two-page document is required by our sports trainers and explains North's concussion management program. It also contains a disclosure form regarding any head injuries that have/have not happened to your student athlete. Requires signature of student athlete and parent/guardian. One form signed per year for each athlete.

Parent & Athlete Agreement - Related to Concussion Law 2011 - Wisconsin Act 172
As a Parent and as an Athlete it is important to recognize the signs, symptoms, and behaviors of concussions. By signing this form you are stating that you understand the importance of recognizing and responding to the signs, symptoms, and behaviors of a concussion or head injury. This form must be completed for every sports season and every youth athletic organization the athlete is involved with. It requires the signatures of both student athlete and parent/guardian.
